Two-Week Continuity Rate = outcomes completed to the agreed standard without founder intervention ÷ outcomes expected during the period.
Report the numerator and denominator with the percentage. This is a management measure, not a universal benchmark. Small counts, a quiet period or vague standards can distort it.
Review failures qualitatively: missing ownership, unavailable information, unclear authority, a broken handoff or an unadopted route.
